The Terros Health justice health clinics are co-located at our Health Centers around the Valley. We work closely with Maricopa County Adult Probation to provide whole health care to meet the physical and mental health needs of the justice community. Individuals have the opportunity to meet with their probation officers, our medical providers, and our mental health professionals in a health care environment focused on your well-being.

What to expect at our
justice health clinics.

Our Health Centers and justice clinics are staffed by a Terros Health nurse practitioner and registered nurse case manager along with counselors and peer community health workers. These are individuals who are specially trained in working with or who have lived experience within the justice community. We provide whole person care. We help connect individuals to the resources they need, from housing and food to transportation and work.

Thinking for a Change (T4C)

We provide our justice community with enrollment in Thinking for a Change (T4C). T4C is a cognitive–behavioral therapy program that helps reshape decision-making, and promotes social skills development and problem-solving skill-building.

Transportation

We can assist in locating or providing transportation to your health care appointments.

Peer Support

Lived experience is a critical aspect of our Justice program. Our staff have served in different areas of the justice community, and our community health workers have walked a mile in your shoes. We provide care, understanding, and support by people who know how best to help.

Resources

Your care team will assist you with accessing necessities for everyday life, including obtaining replacement identification documents and temporary housing, and applying for AHCCCS, food resources, and a cell phone. They can connect you with other community resources and provide you with on-going care.

How do I get started?

If you are a current Terros Health patient who is on probation/or have a client who is a Terros Health patient and on probation, and would like to learn more about resources available to you- contact our care team by calling 480-466-6337 or emailing TIP@terroshealth.org.

If you are a Probation Officer and would like to refer one of your clients to Terros Health, please complete the Referral form and send it (along with an ROI) to the TIP@terroshealth.org email.

What if I need help with transportation?

Our team can assist with transportation resources for medical appointments, as well as for community resources, on a case by case basis.

What’s expected of me?

We are here to provide you with as much or as little assistance as needed based on you or your client’s personal situation.
There is no limit on how often you turn to us for help. We have professional staff available Monday through Saturday. We can meet either face to face, virtually via WebEx, or over the phone.

We work with you to create a treatment plan with goals to help you prepare for your reintegration.

What if I don’t have insurance?

Our team can assist with connecting you to one of our Terros Health eligibility specialists.

What if I need help locating housing, work, transportation, identification and other reintegration aspects?

Our care team can assist with a lot of aspects of reintegration including:

-Assist with applying for AHCCCS, Food Stamps, SMI evaluation and SSI/SSDI

-Thinking for Change program co-facilitated with Maricopa County Adult Probation Dept.

-Obtaining replacement ID, SS card and birth certificate

– Applying for Lifeline cell phone

-Employment & Job Training
-Translation Services
-Scheduling Appointments
-Transportation Assistance
-Referral to GED and Literacy Programs

-Assist in finding temporary housing

-Emergency food boxes

What if I don’t have a doctor for primary health care?

Our team can help you setup an appointment with one of our Primary Care Providers. We can also help answer any questions you have about what to expect during your visit, how to connect to an appointment via telehealth, and more.

Can you pick people up from jail?

Yes, on a case by case basis our team can provide transportation upon jail release. Please contact our care team for more information.
