Integrated, Co-Located Centers Improve Probationers’ Access To Physical and Mental Health Services, Reduce Recidivism.
Terros Health is excited to share that, together with Maricopa County Adult Probation Department, we have received the National Association of Counties (NACo) Achievement Award in Health, recognizing “Innovations in Care: TIP Justice Health Clinics” for our Targeted Investments Program (TIP) Justice clinics. This is a national award for a first-in-the-nation program which was designed to improve probationers’ access to integrated health services and to reduce recidivism.
The program focus is on adults on probation, a population that has experienced inequities in health coverage and care even while experiencing significant health care needs. Our justice clinics have co-located health services and probation offices in four locations in the Valley, including a medical clinic inside of a probation office in west Phoenix.
“The justice involved have historically been underserved when it comes to healthcare. We are changing that dynamic by removing barriers to healthcare through co-locating with Terros Health,” said Michael Cimino, Chief for Maricopa County Adult Probation. “The justice clinics enable probation officers and health care staff to work collaboratively in helping those on probation engage in health care services. We’re taking a more holistic approach and changing the culture, not only with our staff, but for individuals on probation. They are beginning to see that we care about their health and well-being, not just their probation status.”
There are approximately 3,200 individuals reporting to the Department’s Black Canyon Adult Probation Center. Within the initial year of service by our TIP Justice clinics, 1,856 probationers received integrated care at the co-located facilities.

Each center has a Terros Health nurse practitioner and a registered nurse case manager who are specially trained in working with the probation population, along with two counselors, a peer community health worker and a project manager. Our peer community health workers are individuals with lived experience and offer a unique understanding, perspective, and support.

Individuals undergo a comprehensive physical and mental health examination that includes BMI, heart function, diabetes, anxiety and depression screenings. Results from the screenings, along with an evaluation of the individual’s social determinants of health, are used to help develop a customized treatment plan.

In addition to health care, the care team works to identify resources to assist in day-to-day necessities from obtaining identification to locating food and housing.

Terros Health has a long history of serving the probation population, but this program makes health care more accessible to probationers, according to Peggy Chase, the organization’s president and CEO. “We want probationers to feel welcome and know that their health and well-being are valued,” she said. “This program offers coordination of care under one roof, and the opportunity to meet probationers’ physical and mental health needs at one time, giving them a new path to succeed.”

The justice clinics are funded by a five-year grant from the Arizona Health Care Cost Containment System’s Targeted Investments Program. AHCCCS established the program in 2017 to incentivize eligible providers that meet certain benchmarks for integrating and coordinating physical and behavioral health care for Medicaid beneficiaries.

Established in 1972, Maricopa County Adult Probation Department (MCAPD) is part of the Judicial Branch of Arizona within Maricopa County. MCAPD has 19 regional offices and 1,200 employees.
